1. Account Registration

By creating an account, you agree to provide accurate and complete information. You are responsible for maintaining the confidentiality of your account credentials.

You agree to use the E-Request System only for lawful purposes and in accordance with these Terms and Conditions. You must not use the service in any way that could damage, disable, or impair the system.

3. Document Requests

All document requests are subject to approval by the registrar office. You are responsible for providing accurate information and required documents. Fees may apply for certain document requests.

4. Privacy and Data Protection

Your personal information will be handled in accordance with the Data Privacy Act. We will not share your information with third parties without your consent, except as required by law.

5. Payment

Payment for document requests must be made at the USTP Balubal Cashier. All fees are non-refundable once the request has been processed.

6. Account Termination

We reserve the right to suspend or terminate your account if you violate these Terms and Conditions or engage in any fraudulent activity.

7. Limitation of Liability

The University of Science and Technology of Southern Philippines shall not be liable for any delays, errors, or omissions in the processing of document requests beyond our reasonable control.

8. Changes to Terms

We reserve the right to modify these Terms and Conditions at any time. Continued use of the service after changes constitutes acceptance of the new terms.
